Registration

Accreditation
This activity has been planned and implemented in accordance with
the Essential Areas and Policies of the Accreditation Council for
Continuing Medical Education (ACCME) through the joint sponsorship
of the Duke University School of Medicine and Philips Medical
Systems. The Duke University School of Medicine is accredited by the
Accreditation Council for Continuing Medical Education (ACCME) to
provide continuing medical education for physicians.

Credit Statement
The Duke University School of Medicine designates this educational
activity for a maximum of four hours in Category 1 credit towards the
AMA Physician’s Recognition Award. Each physician should claim
only those hours of credit that he/she actually spent in the educational
activity.
This activity has also requested four credit hours from the SDMS.

Registration Fees
Physicians $150
Sonographers, Residents, Fellows $110
(Fee includes tuition, instructional materials, lunch, refreshments, and
SDMS or CME credits.)

Receiving Confirmation
After we receive your registration form and fee, we will send you a
confirmation letter containing information about, and directions to,
your conference site. Please note that online registrations will immediately receive an e-mail confirmation. However, if the online form of
payment was by check or coupon, the confirmation letter will be sent
only after the check or coupon has been received.

Deadline
Registration ends January 28, 2002. Due to seating limitations, we
encourage early registration
. Many sites are filled before the deadline
date. There will be NO ON-SITE registration. We will accept registrations
after January 28, 2002 on a space-available basis. To confirm
late registrations, call the Teleconference Information desk at
1-850-784-6002, ext. 15.

Cancellation
If you register and are unable to attend, you may send an alternate
from your institution. A full refund, however, will be issued to cancellations
received before January 28, 2002; cancellations received
after this date will not be refunded. To cancel, please call the
Teleconference Information desk at 1-850-784-6002, ext. 15.
